There are several ways you can schedule an appointment at Surgical Associates, either by telephone or on-line. To request an appointment with one of the Surgical Associates physicians, or to request a change in an appointment you have already scheduled, click here. A staff member in charge of scheduling will review your request and contact you by telephone or on-line with details of  your appointment.

To speak directly with scheduling personnel, please telephone our office at (918) 481-4800 between the hours of 8 a.m. and 5:30 p.m. Monday through Friday. Each physician is scheduled in the office at certain times during the week. While you are on the telephone, our staff will advise you of these times in order to make the most convenient appointment for you.

We will make every effort to provide an appointment with the surgeon you or your doctor request. If no preference is indicated, an appointment is made at the first available time that is convenient for you. Subsequent appointments are made with the same doctor to provide the best possible continuity of care.

In certain cases, your doctor may order x-rays or lab work which will need to be done outside of our office prior to your visit. Your doctor may also want to view other existing x-ray films and reports at the time of your visit, particularly mammograms. Please ask the receptionist when you make your appointment if you need to bring these studies from your radiologist's office.

It will be necessary to complete some paperwork on your first visit to our office. For your convenience, this can be done in advance of your visit. Click here to obtain copies of these forms that you can fill out online and print out prior to your appointment.
