After each visit to Surgical Associates, you will receive an itemized bill which shows all charges for your care, along with insurance codes. For office visits, either payment or co-payment is due at the time of service. If your insurance requires an authorization from your primary care physician prior to being seen by one of our physicians, it is your responsibility to obtain that authorization. We will file insurance for office visits if you are enrolled in a PPO or HMO in which we participate.

For other types of insurance, in most cases, you will need to fill out your part of the insurance claim form, attach our bill, and send it in for reimbursement.

We automatically file with your insurance company for all hospital services, no matter which insurance company you may use. Please keep us informed of the insurance plan you have, and be sure to let us know if your insurance or insurance coverage has changed since your last visit. Notify the Medical Assistant if your insurance company has special requests such as second opinion, or precertification prior to your surgical care.

Our surgeons also participate in the Medicare program. This means we will accept Medicare's allowable payment and you will only be billed for the 20 percent co-insurance and any deductible taken from our payment.
